Effective June 13, 2001 the Florida Statutes governing IPC filing have changed. All IPC policies with effective dates after
June 13, 2001 are required to comply with the changes to the statutes (F.S. 626.938).
Effective September 1, 2001, FSLSO will be collecting the required IPC policy information via this website.
The Florida Department of Financial Services forms formerly utilized will no longer be accepted.

To facilitate the information submission process, it is recommended that you have the
following information readily available before you begin. We have made a
presubmission worksheet available (PDF) to aid you in
compiling the information needed prior to submitting your data. Please do NOT submit this worksheet with your invoice.:
- Your contact information (i.e. name, address, phone, email)
- Insured Entity information (i.e. name, address, phone)
- The type of coverage for the policy being submitted (descriptions will be provided)
- Policy number
- Policy effective date
- Policy expiration date
- County(ies) the risk is located in (see here for info)
- Total premium amount of the policy
- Coverage amount if policy is Residential Property
- Insurer Name (FEIN and address are required if the insurer is not on the FSLSO Insurer list)
- You will also need to make sure that you have a printer available, and at least version 5.0 of
Adobe Acrobat Reader installed properly on your system prior to beginning the data submission process.

The Florida Surplus Lines Service Office's (FSLSO) duties, with respect to the Surplus Lines Law, include:
- collecting data on surplus lines business written in the state of Florida from Surplus Lines Agents and IPC filers,
- comparing this data to the information filed by the Surplus Lines Insurers, and
- billing and collecting taxes and fees due on this business and remitting the taxes to the Florida Department of Financial Services.
Florida Statute 626.938 governs this action. The law requires an insured, who
procures insurance coverage directly from unauthorized insurers (rather than through a
Florida licensed and appointed surplus lines agent), to report the policy data and premium
to the FSLSO using a computer format designated by the FSLSO. The insured is required to
pay a service fee
and a premium receipts tax. Please see Florida Statute
626.938 for more information.
FSLSO's method for collecting this data will be as follows:
- IPC filers will log on using the link below
- then enter the required identification information and submit via a submit button
- IPC filers will receive an email containing an access link to the FSLSO IPC submission form
- after clicking on the link provided, the IPC filer will input and submit the policy data to FSLSO
- FSLSO will provide an opportunity for the filer to verify data and print the two required invoices using Adobe Acrobat
- the IPC filer will mail each invoice along with an appropriate check. Taxes will be mailed to the Florida Department of Financial Services and the service fee being mailed to the FSLSO.
