AGENT ADDRESS CHANGE
The FSLSO should be informed within 30-days if there is a change of address or other pertinent information. Please send to the FSLSO a revised FSLSO Membership Information Form when your agency''s address, phone number, fax number and/or email address changes.
Also, it is very important that you make the changes in your Surplus Lines Agent Software. In the software you will revise both the Agency and Surplus Lines Agent sections under "Setup", and the Contact section under "Utilities". The information submitted through your agent software overrides address changes made by FSLSO, if you fail to revise the software your confirmations, invoices, etc. will be sent to the old address. Revising the Agent Software immediately will ensure you receive correspondence from FSLSO in a timely manner.
