FSLSO Encourages Going Green: Look for Your Electronic Invoice

In an effort to promote electronic invoicing, on Thursday, May 1, 2008, our office will be sending out ALL invoices and detail reports via email to the submission contact person that made the last submission to our office. All invoices will be sent as .pdf attachments and the detail reports will be sent in a separate email as an excel spreadsheet attachment in a .zip file.

Please note that if you normally receive your invoices and detail reports through the mail, you will continue to receive them through the mail unless you notify our office requesting to change to electronic invoicing.

If you do not currently receive your invoices electronically, but would like to take advantage of this service, please send an email to Jennifer Mills (jmills@fslso.com) or Sheila Pearson (spearson@fslso.com). Please include the agent name, license number, submission contact name and email address.


Quarterly Report Affidavit Due TODAY!

The first quarter of 2008 ended on March 31 which means your Quarterly Report Affidavit is due TODAY!

If you have not filed your affidavit at this time, you will need to file it through SLIP in order to meet the deadline. Never filed your affidavit through SLIP? No problem! Filing through SLIP is simple, quick and efficient.

How do I file my affidavit electronically?

1) Login into SLIP using your username and password. (Don't have one, call FSLSO for yours today! (800) 562-4496 extensions 104, 106 and 129)
2) After logging in, select the "Reports" box in the top left portion of the screen.
3) Next, select "Quarterly Affidavit Report" in the report box.
4) Then select the appropriate quarter and year in the corresponding boxes.
5) Select "Retrieve Premium" - the affidavit will then appear upon selecting "Retrieve Premium."
6) Then, you will need to select your agency from the agency drop-down box.
7) Notice the system calculated premium amount at the bottom of the screen. If you agree with the calculated amount, simply check the "I acknowledge the terms and conditions" box.
8) If you don't agree with the amount, please provide your calculated amount in the "Your Calculated Amount" box, and then check the "I acknowledge the terms and conditions" box.
9) Lastly, select the "Submit" button - your affidavit will be sent to FSLSO at that time.
10) After submitting the affidavit, you may also select the "Print" button to print a copy for your records.

Rule 690-167.004 Preinsurance Inspection of Private Passenger Motor Vehicle

Are you writing private passenger motor vehicles physical damage coverage? If so, are you aware of the requirement outlined in Rule 69O-167.004 for preinsurance inspection of private passenger motor vehicles? The rule requires insurers providing physical damage coverage, including collision or comprehensive coverage to comply with Section 627.744, Florida Statute, regarding preinsurance inspections was adopted in 1991 and amended in January, 2008 to comply with amendments to the statute and update the forms.

Section (7) of the rule outlines the surplus lines agents’, responsibility if they write physical damage coverage on private passenger motor vehicles with an eligible surplus lines insurer:
(7) The preinsurance inspection form, or an electronically or photographically reproduced copy, shall be retained by the insurer with the insured’s policy records at the insurer’s home office, regional office, or district office for a period of three (3) years. When the insurer is a surplus lines company, these records shall be kept in the Florida office of the surplus lines agent for that insurer. The original signed preinsurance inspection form shall be maintained by the insurer/surplus lines agent and shall be made available to the Office upon request. A copy of the inspection form, without any optional accompanying photographs, shall be made available to the insured upon request.


Attention Alien Insurers! Annual Premium Submission Reminder

For all alien insurers, an electronic premium report is due June 30, 2008. The annual report of policy information for policies issued during the year 2007(Jan-Dec) should be submitted electronically and include: policy number, effective date, premium amount and name of insured. In the event there is nothing to report, the insurer is still required to submit a "No Business" letter in lieu of the report.

Pursuant to Florida Statute 626.9361, a $500 fine per day for each day the failure continues applies to any eligible surplus lines insurer who fails to file a report in the form and within the time required as provided for in the Surplus Lines Law. This begins the day after the report was due and until the date the report is received. Failure to file a report may also result in withdrawal of eligibility as a surplus lines insurer in Florida.
